Medicare GLP-1 pilot excludes 5.9 million enrollees with qualifying diagnoses

An estimated 5.9 million Medicare enrollees are excluded from a pilot program offering $50 monthly GLP-1 medications due to specific program design rules. While the Bridge program aims to provide affordable weight loss drugs, it is restricted to beneficiaries using the medication solely for obesity.

Under the current framework, individuals with qualifying diagnoses such as Type 2 diabetes, moderate to severe obstructive sleep apnea, or metabolic liver disease are routed back to standard Medicare Part D plans. Because these patients have existing coverage routes for these conditions, the pilot excludes them to avoid duplicating Part D benefits. This creates a coverage inversion where a patient treated only for obesity may pay $50 per month, while a sicker patient with additional qualifying conditions may face copays totaling hundreds of dollars.

CMS has described the 18-month pilot as a test to determine if early obesity treatment can reduce long-term program costs. KFF analysis indicates that of the 9.7 million Part D enrollees meeting clinical criteria, fewer than 4 million remain eligible after these exclusions are applied.
