The Centers for Medicare & Medicaid Services reported that in fiscal year 2025 its new prevention‑first strategy saved $42 billion by stopping fraudulent claims before payment, a 60 % increase over the prior year. Nearly 70 % of those savings came from revoking providers and intercepting payments at the point of submission, delivering more than $22 saved for every dollar spent on program integrity.
