Nayoro healthcare professionals discuss heart failure care models

Healthcare and social welfare professionals gathered at Nayoro City General Hospital for the third Care Cafe Nayoro event, a collaborative initiative focused on heart failure management. The meeting brought together 54 participants from medical, nursing, fire, administrative, and social welfare sectors to discuss multi-disciplinary cooperation.

During the session, physical therapist Sayaka Kakikawa presented research on elderly heart failure patients, noting that the ‘Nayoro Model’ of medical and nursing collaboration helps maintain the self-care abilities of patients and prevents condition worsening. Nurse Shiran Miyakoshi shared a case study of an elderly male patient to highlight the challenges of reaching individuals who decline public services despite needing home-based nursing or pharmaceutical intervention.

Participants discussed expanding support through peer involvement and Advance Care Planning (ACP) to respect the intentions of patients and their families. Dr. Hiroshi Sakai emphasized the necessity of face-to-face communication among supporters to expand the circle of cooperation between professionals and non-professionals.
