New York City’s public health system is preparing for potential funding reductions due to a proposed revision by the Office of Management and Budget (OMB). The proposed changes to the ‘Uniform Guidance’ rules would grant the Trump administration broader discretion to award, manage, and terminate federal grants with minimal explanation or opportunities for appeal.

Currently, federal grants cannot be canceled without cause. The proposed rule change could significantly impact the New York City Department of Health and Mental Hygiene and various community-based health centers. Approximately $500 million, representing 20% of the city’s Health Department budget, is provided by the federal government and is designated for disease surveillance and emergency preparedness.

Health experts, including Dr. Oni Blackstock, have noted that while the changes might not completely disrupt operations, they could destabilize the core functions of the Health Department. The potential impact of these OMB revisions is expected to extend beyond New York City to states and counties across the United States.
