Nicotine addiction vs. tobacco combustion health risks

Public health discussions are increasingly distinguishing between the addictive properties of nicotine and the lethal effects of tobacco combustion. While nicotine is the primary driver of addiction due to its ability to release dopamine and restructure brain chemistry, it is the smoke produced by burning tobacco that introduces thousands of toxic chemicals into the body.

Research indicates that the combustion process releases at least 70 known carcinogens, including formaldehyde, benzene, arsenic, and hydrogen cyanide. These substances, which are not present in nicotine itself, are directly linked to serious diseases such as cancer.

In the context of harm reduction, experts suggest a “hierarchy of harm” regarding nicotine delivery methods. While no nicotine product is risk-free, the cardiovascular risks vary depending on whether the nicotine is inhaled via smoke, vapor, or delivered through non-combustible methods like patches, gum, or pouches. The FDA has recently authorized new nicotine pouches that contain lower levels of harmful constituents compared to traditional smokeless tobacco products.
