Nurse-led interventions reduce heart failure readmissions
A rapid review of studies from the United States, Japan, Brazil, and Canada indicates that nurse-led transitional care interventions can help reduce hospital readmissions for heart failure patients. Heart failure affects more than 64 million people globally, with 20-25% of patients being readmitted within 30 days of discharge due to gaps in care, such as poor communication and inadequate follow-up.
The review found that multicomponent interventions—including continuous follow-up, symptom monitoring, and care coordination—are more effective than single-component approaches like discharge education alone. Specifically, telemonitoring has been shown to reduce rehospitalization and improve patient engagement.
