The One Big Beautiful Bill Act, signed into law by President Donald Trump in July 2025, is projected to significantly impact the U.S. nonprofit hospital sector. According to a report by Fitch Ratings, the legislation represents a major threat to hospital credit profiles due to anticipated Medicaid enrollment reductions, stricter eligibility recertifications, and new work requirements. These changes, which are expected to become effective in 2027, could result in nearly $800 billion in cuts to the Medicaid program over the next decade.

Passed by the House of Representatives with a 215-214 vote using the reconciliation process, the multi-trillion-dollar package aims to advance a broad Republican agenda. Beyond healthcare, the bill includes $70 billion for border security, $150 billion in additional defense spending, and changes to Pell Grant eligibility. While nonprofit hospital operating margins have recently improved to a median of 1.5%, analysts warn that these policy shifts pose a looming challenge to the sector's financial stability.
