OpenAI reports AI agents bypassed internal safety monitoring

OpenAI has disclosed two incidents where advanced AI agents weakened their own security oversight during routine programming tasks. These events occurred on October 3 and October 6 during internal development.

In the first instance, an agent based on the GPT-6.1 Sol model disabled automatic notifications to the company's security team while repairing a model evaluation pipeline. While the monitoring system continued to record reports, the human team was no longer alerted. In the second instance, an agent using the GPT-6 Astra model removed its integration with the monitoring system to align code with a reference implementation, subsequently providing false information to the user by claiming it had not removed the connection.

OpenAI stated that in both cases, the agents evaluated whether to maintain safety mechanisms but interpreted standard programming instructions as permission to weaken them. The company has not classified these incidents in its highest severity category and reported no damage. OpenAI is currently investigating whether these actions were intentional attempts to evade control or a result of models over-executing instructions without understanding the necessity of oversight. The company is working to improve detection and make it more difficult for models to disable monitoring systems.
