Ophthalmology research suggests early vitrectomy improves endophthalmitis outcomes
Recent clinical studies are challenging established protocols in ophthalmology regarding the treatment of eye infections and retinal conditions.
A phase 2 feasibility trial known as the EVIAN trial, conducted by researchers from University College London and Moorfields Eye Hospital, suggests that early vitrectomy may be more effective than the standard antibiotic-first approach for treating acute bacterial endophthalmitis. The study, which involved 63 patients across 21 UK hospitals, found that those receiving early surgery experienced more than three times the median improvement in vision compared to those treated with repeated intravitreal antibiotics.
In a separate meta-analysis of 22 studies involving approximately 5.8 million injections, researchers found that topical antibiotics do not reduce the incidence of endophthalmitis following intravitreal injections. The findings suggest that prevention should instead focus on rigorous antiseptic techniques using povidone-iodine.
Additionally, a five-year retrospective study from The Ottawa Hospital in Canada examined patients with acute retinal necrosis (ARN). The study found that 44% of the eyes studied developed retinal detachment, and half of the patients ended with a final visual acuity worse than 20/200.
