Optus network outage linked to four deaths in Australia

A major Optus network outage caused by a botched firewall upgrade has been linked to at least four deaths in Australia. The disruption, which lasted approximately ten hours in September 2026, affected South Australia, Western Australia, and the Northern Territory, preventing roughly 600 attempts to reach the Triple Zero emergency service.

Authorities confirmed the fatalities included a 49-year-old man and a 74-year-old man in Perth, as well as a 68-year-old woman and an eight-week-old baby in Adelaide. Optus CEO Stephen Rue stated the failure went undetected by automated monitoring systems and apologized for the incident.

The outage follows a series of reliability concerns regarding Australia’s telecommunications infrastructure, including a significant Telstra failure earlier in the year. Federal regulators and Communications Minister Anika Wells have promised investigations and accountability following the disruption and delayed public notifications.
