PBM reform and transparency trends impact prescription drug costs

The pharmacy benefit management (PBM) industry is facing increasing pressure for transparency as plan sponsors seek better control over prescription drug costs and data. Industry experts note a shift toward newer PBM models that provide clearer fee disclosures and greater access to information regarding claims and pricing.

However, analysts caution that legislative and regulatory reforms may not automatically result in lower costs for employers. While Congress and agencies like the Federal Trade Commission (FTC) are scrutinizing PBM revenue streams and vertical integration, there is a risk that revenue may simply shift to other entities, such as specialty pharmacies or rebate aggregators, rather than being eliminated.

The FTC has previously documented significant revenue generation from affiliated pharmacies, and the U.S. Department of Labor has proposed disclosure rules to give fiduciaries more visibility into PBM compensation. Experts suggest that for reform to be effective, plan sponsors require comprehensive transparency across the entire healthcare arrangement to ensure real savings are achieved.
