Pharmacists play key role in preventing home-based medication errors
On World Patient Safety Day, Nazila Yousefi, Director General of the Directorate General of Monitoring and Surveillance of Health Product Consumption, highlighted that a significant portion of medication errors occur at home after patients leave the pharmacy.
Citing World Health Organization data, Yousefi noted that one in ten patients globally experiences harm during healthcare delivery, with nearly half of these incidents being preventable. Medication and treatment-related injuries constitute a substantial share of these preventable harms.
Yousefi emphasized the critical role of pharmacists in identifying potential errors, checking for drug interactions, and providing counseling, particularly for patients with chronic conditions like diabetes and hypertension. She urged all medical professionals to report adverse drug reactions to the official registration center to support pharmacovigilance efforts.
