The game features two protagonists, Jason and Lucia, whose relationship and shared activities are central to the narrative. Developers originally considered a four-character structure but opted for a duo to ensure deeper emotional connections with players.

The game world, set in the state of Leonida and featuring Vice City, is massive; the map is reportedly twice the size of GTA V and three times the size of Red Dead Redemption 2. Gameplay mechanics include a Criminal Profile system that tracks player behavior, a multi-layered vehicle theft system, and RPG-style character customization where diet and exercise can physically alter the protagonists' bodies.

Economically, Rockstar has clarified that the main single-player campaign will not include microtransactions. Additionally, the studio confirmed that generative AI was not used in the development process, emphasizing manual craftsmanship.
