Romania health insurance costs for non-earners in 2026
In Romania, individuals without income who are not otherwise insured can opt to pay the Social Health Insurance Contribution (CASS) to access the public healthcare system in 2026. According to ANAF, the total cost for 12 months of insurance is 2,430 lei.
This amount is calculated based on the gross minimum wage effective on January 1, 2026, which was 4,050 lei. The calculation uses the equivalent of six minimum wages (24,300 lei) with a 10% contribution rate. Although the gross minimum wage increased to 4,325 lei on July 1, 2026, this change does not affect the CASS amount for the year, as the calculation remains tied to the January 1 rate.
The contribution can be paid in two installments. Once the 'Declarația unică' (Single Declaration) is submitted, the individual is insured for a period of 12 months from the date of filing, meaning coverage does not automatically expire at the end of the calendar year.
