Salut Sant Joan launches pilot to prevent medication risks in elderly

Salut Sant Joan de Reus-Baix Camp has launched a pilot program as part of the European STOP-IATRO project to prevent adverse health effects caused by medication in elderly patients. The initiative focuses on approximately 50 patients over the age of 75 who suffer from cardiovascular diseases and polypharmacy, defined as the regular consumption of five or more medications.

The pilot will take place at the Intermediate Care Home Hospitalization Unit (HADI) of the Hospital Universitari Sant Joan de Reus. Led by the Primary and Community Care Pharmacy Service (SEFAPiC), the project utilizes an interdisciplinary committee including specialists in geriatrics, internal medicine, cardiology, and nursing to review and optimize pharmacological treatments.

The study aims to analyze how structured medication reviews impact excessive medication use, potentially inappropriate prescriptions, fall risks, and patient functionality. By addressing the physiological changes associated with aging, which can alter drug responses, the project seeks to reduce avoidable hospitalizations and the loss of patient autonomy.
