Seoul St. Mary's Hospital performs Korea's first LLAMACORN TAVI valve‑in‑valve procedure

Seoul St. Mary's Hospital successfully treated an 89‑year‑old woman with a second transcatheter aortic valve implantation (TAVI) using the LLAMACORN (LLAMACORN) technique. The patient had received her first TAVI six years earlier, but the implanted valve had degenerated and caused severe aortic stenosis.

The LLAMACORN method pre‑cuts the leaflets of the existing valve to keep the coronary artery opening clear before inserting a new self‑expanding valve, reducing the risk of coronary obstruction that can lead to heart attack or cardiac arrest. The procedure avoided open‑heart surgery, offering a less invasive option for high‑risk elderly patients. Hemodynamic testing the day after showed the peak blood‑flow velocity through the aortic valve dropped from 4.6 m/s to 2.1 m/s, indicating improved flow.

This was the first domestic use of the LLAMACORN technique in South Korea; the method had previously been reported only in an Australian study earlier in 2024. The intervention was led by Professor Jang Ki‑yook and his cardiovascular team, marking a milestone for the hospital’s interventional capabilities.
