Singapore hospital model uses AI to reduce readmissions and costs
Singapore General Hospital (SGH) utilizes a Discharge & Support Hub model to manage high-risk patients and reduce medical costs. The system focuses on two primary groups: patients with chronic illnesses who are at high risk of readmission, and those who lose contact with medical facilities after discharge.
The model is built on three pillars, starting with the early identification of high-risk patients in the Emergency Department. To enhance this process, SGH has collaborated with the SingHealth AI Office to develop predictive tools. These tools automatically identify priority groups, such as elderly patients from nursing homes, individuals with psychosocial difficulties, or those requiring end-of-life care.
By ensuring continuous care and preventing patients from being lost between treatment stages, the model aims to reduce hospital bed occupancy and the significant financial burden placed on families and the healthcare system.
