Spain private health insurance usage grows 81% since 2014

The number of people using private health insurance in Spain has grown by 81% since 2014, now accounting for 21% of the population, according to the 12th Report on Healthcare Privatization by the Federation of Associations for the Defense of Public Health (FADSP).

Report coordinator José Manuel Aranda noted that family out-of-pocket spending has increased by 52.8%, averaging 576 euros per family. FADSP secretary Sergio Fernández stated that the primary driver for this trend is delays in the public healthcare system, specifically regarding primary care appointments and specialist consultations.

In 2025, private healthcare spending reached approximately 34 billion euros, representing 2.5% of Spain's GDP and roughly 27% of total national healthcare expenditure. The regions with the highest levels of service privatization are Madrid, the Balearic Islands, Catalonia, and the Canary Islands.
