Stroke rehabilitation guidelines emphasize early intervention and cognitive care
New clinical guidance and medical expert perspectives emphasize that stroke rehabilitation must be integrated into the acute care pathway rather than being delayed until after hospital discharge.
The American Stroke Association has released updated adult stroke rehabilitation and recovery guidelines, replacing the previous standards from 2016. The new guidelines recommend that rehabilitation begin as soon as a patient is medically stable, ideally within 48 hours. Notably, the updated approach shifts focus from purely physical function to include cognition, communication, and mental health, such as depression screening.
While early assessment and safe mobility are encouraged on the first day, the guidelines advise that moderate to high-intensity exercise and task practice should not be performed within the first 24 hours. Experts suggest that a multidisciplinary approach—involving neurologists, physiotherapists, speech specialists, and psychologists—is essential to address diverse needs such as swallowing, memory, and balance to improve long-term functional outcomes.
