Swedish municipalities face rising costs for personal assistance

Swedish municipalities face significant financial pressure due to declining state reimbursement for personal assistance services. Two-thirds of private and non-profit assistance providers report they may be forced to close or exit the industry within the next year unless compensation is increased.

If private providers exit, municipalities may be forced to take over all assistance services. This shift threatens the self-determination of individuals with disabilities and poses a major fiscal risk. Grant Thornton has noted that municipal costs for personal assistance are, on average, 30 percent higher per hour than the state's reimbursement rate. This discrepancy could result in a total national additional cost of 10 billion SEK for municipalities.

Specific local impacts include Kristianstad Municipality facing 129 million SEK in extra annual expenditures, Östra Göinge Municipality facing 17 million SEK, and Bromölla Municipality facing 12 million SEK. Proponents suggest that a state increase of at least 6 percent in compensation by 2027 would cost approximately 1.2 billion SEK, a figure significantly lower than the potential 10 billion SEK municipal burden.
