Tesla driver-assist system active in fatal Arizona freeway crash

Data submitted by Tesla to the National Highway Traffic Safety Administration (NHTSA) reveals that a 2020 Model 3 was operating with a Level 2 driver-assist system engaged during a fatal accident in Mesa, Arizona. The incident occurred on October 31, 2025, when the Tesla came to a complete stop in a freeway lane and was subsequently struck from behind by a Ford F-350 pickup truck, resulting in the driver's death at the scene.

This incident follows a similar fatal accident in Florida involving a 2020 Model 3 that also stopped in a live interstate lane while driver-assist systems were active. In the Arizona case, Tesla's records indicate the vehicle's engagement status was ‘Verified Engaged’ and its speed was ‘0 mph’ at the time of impact, despite clear weather and normal road conditions.

While the data confirms the automation was active, it remains unclear whether the system caused the vehicle to stop or if other factors, such as driver health or ‘phantom braking,’ were involved. Tesla reportedly possesses the event-data recorder, telematics, and video footage necessary to determine the exact cause of the stoppage.
