Turkey introduces new performance coefficient for physicians
The Turkish Ministry of Health has introduced a new Individual Target Coefficient system that will affect the incentive payments of physicians, including doctors, dentists, and specialists. The coefficient, which ranges between 1 and 1.4, is applied to incentive extra payments based on performance indicators rather than fixed salaries or base payments.
Under the new model, a physician's coefficient is determined using a Z-score system. This means a doctor's score is not based solely on their own output, but is compared against the scores of colleagues in the same specialty working in health facilities of similar quality and type. During the transition period in August 2026, the coefficient will be set at 1 for all physicians.
The Genel Sağlık-İş union has criticized the regulation, arguing that healthcare services cannot be reduced to performance points. The union expressed concerns that the system will force healthcare workers into competition with one another, potentially damaging workplace peace. Critics also suggest the model may pressure physicians to prioritize the number of procedures and points over medically necessary care, such as detailed patient consultations or avoiding unnecessary interventions.
