Federal judge blocks Trump administration Medicaid rule

U.S. District Judge Richard Stearns has blocked a rule from the Centers for Medicare & Medicaid Services (CMS) that would have required chronically ill Medicaid recipients to prove their conditions significantly impair their ability to work in order to qualify for medical exemptions.

The ruling, issued in Boston, provides a partial victory to a coalition of 25 states and the District of Columbia. The plaintiffs argued that the administration's stricter definition of “medically frail” overstepped the authority granted by the 2025 One Big Beautiful Bill Act and would prevent vulnerable individuals from accessing necessary care.

While the judge blocked the expanded impairment test, the decision does not strike down the Medicaid work requirements themselves. Under the law, adults aged 19 to 64 must complete 80 hours of work or community engagement per month starting January 1, 2027. Stearns also upheld two other federal restrictions: states may reject medical frailty proof if a diagnosis is more than a year old, and states may accept only one self-attestation per continuous enrollment period.
