Study Links Tap Water Disinfection Byproducts to Higher Uterine Cancer Risk

A long‑term observational cohort of 53,100 California teachers and administrators was followed from 1995‑1996 through the end of 2020. During the follow‑up 1,038 women developed uterine cancer. The analysis found that women living in areas with the highest long‑term exposure to total trihalomethanes (THMs) — chemicals formed when chlorine disinfects water — had an overall uterine cancer risk about 18 % higher than those with lower exposure. Within the THM group, chloroform showed the strongest association with endometrioid tumors, which represent the majority of uterine cancers. Separate exposure to haloacetic acids (HAAs) was linked to nearly double the risk of the rarer, more aggressive non‑endometrioid tumors. No significant association was observed for other measured water contaminants such as nitrate, arsenic, or uranium. The study was published in JAMA Network Open and highlighted that while water disinfection remains essential for public health, the resulting by‑products may be a modifiable risk factor for uterine cancer, the most common gynecologic cancer in the United States, with higher mortality observed among Black women.
