ASPIRED trial: Wearable monitors double arrhythmia detection

The ASPIRED trial, conducted across 45 UK hospitals, has demonstrated that using a 14-day wearable ECG monitor for patients with unexplained fainting can significantly improve the detection of dangerous heart arrhythmias. Findings presented at the European Society of Cardiology Congress and published in the New England Journal of Medicine show that the monitor more than doubled the detection rate of life-threatening rhythm problems, identifying them in 22% of patients compared to 9% under standard care.

In addition to higher detection rates, the study found that diagnoses occurred much faster, with a median of 22 days compared to 55 days under standard protocols. The early intervention also correlated with a lower mortality rate; only 1.5% of the monitored group died within one year, compared to 2.9% in the standard-care group.

Professor Matthew Reed of the Usher Institute noted that diagnosing syncope in emergency departments is challenging because intermittent rhythm disturbances often resolve before a patient reaches the hospital. The trial utilized a small, waterproof, leadless device that allowed patients to record continuous data and signal fainting episodes, helping to close the diagnostic gap left by traditional 48-hour Holter monitors.
