White House launches Fraud Ledger to track $230 billion in federal fraud

The White House has launched a new online tool called the ‘Fraud Ledger’ to provide public transparency regarding federal fraud investigations, enforcement actions, and prevention efforts. Since January 2025, the administration reports it has uncovered nearly $230 billion in fraud and prevented approximately $56 billion in fraudulent payments.

Healthcare programs administered by the Department of Health and Human Services (HHS) are a primary focus of the initiative. HHS reported over $90 billion in improper payments for fiscal year 2025, with Medicare and Medicaid identified as major areas of concern. Specifically, Medicaid fraud has been categorized as a ‘highest-impact’ anti-fraud action.

Assistant Attorney General Colin McDonald noted that investigators are utilizing artificial intelligence to combat these issues, including specific efforts to address Medicaid fraud in Minnesota. The ledger tracks three specific metrics: fraud uncovered through data analysis, fraud stopped through administrative actions like provider suspensions, and fraud enforced through indictments and settlements.
