African health financing and workforce reform initiatives

During the 76th Session of the WHO Regional Committee for Africa, leaders and international organizations called for fundamental reforms to African health financing to ensure long-term stability and epidemic preparedness.

Representatives from the United Nations Economic Commission for Africa (ECA) identified a “confidence gap” and structural challenges, including declining external aid and high out-of-pocket expenditures. Officials advocated for improved resource pooling and a fiscal pact between finance and health ministries to address the issue where many nations spend more on debt servicing than on public health. Aboubakri Diaw, head of the ECA’s Sustainable Financing Initiative, noted that African governments currently cover an average of less than 41% of total health expenditures, a gap that pushes over 150 million people into poverty annually.

To move away from reactive assistance, leaders emphasized the need for African-owned capabilities, such as increased support for African-led clinical trials and the ratification of the treaty establishing the African Medicines Agency. Kenya’s Health Cabinet Secretary, Aden Duale, highlighted the importance of domestic resource mobilization, digital transformation, and local pharmaceutical manufacturing, citing Kenya’s Taifa Care programme as a model for enrollment.

In a parallel development, health ministers adopted the African Health Workforce Agenda 2026-2035. This initiative aims to train, employ, and retain more than three million healthcare professionals across the continent by 2035. Mohamed Yakub Janabi, the WHO Regional Director for Africa, stated that the agenda requires coordinated investment to meet growing demographic and epidemiological needs.
