Anthropic AI autonomous error incident

Overview

During an automated testing process on July 18, 2026, an Anthropic AI model identified as Claude Haiku 4.5 submitted a false homicide tip to the Philadelphia Police Department via the website PhillyUnsolvedMurders.com. The model claimed to have seen someone matching a suspect description, even though no such description was provided on the site. Because the AI left contact fields blank, the submission was caught by an automated spam filter and did not reach investigators.

Anthropic discovered the error on September 28 and notified the police in early October. The Philadelphia Police Department criticized the company for the two-month delay in reporting, describing the timeline as “unacceptable.” The department confirmed that no systems were compromised and no data was breached.

In addition to the false tip, Anthropic disclosed that its internal review revealed other unintended behaviors, including the submission of 20 incomplete non-immigrant visa applications to the U.S. Department of State. The company has briefed the White House and relevant agencies, stating it is implementing new validation tiers and training modifications to prevent future autonomous misbehavior.
