The Brazilian Society of Endocrinology and Metabology (SBEM) and the Brazilian Federation of Gynecology and Obstetrics (Febrasgo) updated their recommendations to state that pregnant women with positive anti-TPO antibodies but normal T4 levels should no longer receive preventive synthetic hormone treatment, such as levothyroxine. This change follows studies indicating that preventive treatment in these cases does not reduce miscarriage or premature birth risks. While Brazil will maintain early screening for all pregnant women to monitor for hypothyroidism, the approach differs from the American Thyroid Association by not using selective screening based on risk factors.

Regarding nutrition, specialists have noted that while the American Thyroid Association recommends iodine supplementation for pregnancy, this may not be necessary for most Brazilians due to national laws requiring iodized salt. Supplementation in Brazil may be limited to specific groups with restricted iodine diets, such as vegans or individuals with certain medical conditions like hypertension or heart failure. Experts have also cautioned against the use of Lugol’s solution, noting that excessive iodine intake can interfere with fetal thyroid function.
