French healthcare reform and reimbursement changes

The French healthcare system is undergoing significant structural and financial changes. Initially, a proposed reform aimed to transition the payment model from fee-for-service to a capitation system. This plan, reportedly developed by the President and advisors, suggested an annual subscription model for patients to replace individual consultation fees. Medical professionals expressed concern that this shift could cause a 50% to 70% decrease in income, potentially leading to a mass disengagement from the healthcare system.

Further regulatory changes have since been implemented regarding reimbursements and access. As of October 1, the annual cap for medical franchises increased from 100 to 140 euros per insured person. Additionally, reimbursement rates for certain medications with low medical service rendered were reduced to 15 percent for patients with long-term conditions, though indispensable treatments remain fully covered. To address primary care access, a new decree allows patients in Mayotte and five other departments to consult advanced practice nurses directly without a prior doctor's referral starting December 1, 2026.
