Global second-hand smoke exposure and tobacco control

Overview

A global study published in The Lancet Public Health, part of the Global Burden of Diseases research, indicates that approximately 2.71 billion people, or one in three people worldwide, were exposed to second-hand smoke in 2023. This exposure included 767 million children under the age of 15 and 1.49 billion women, contributing to an estimated 1.66 to 1.7 million deaths.

While the age-standardized prevalence of exposure has declined by 22.2% since 1990, the absolute number of affected individuals remains high due to population growth and unequal implementation of tobacco control policies. Regional disparities are notable, with exposure nearly doubling in Sub-Saharan Africa and increasing by 72.1% in North Africa and the Middle East, while Latin America and the Caribbean saw the largest declines. Medical experts emphasize that “there is no safe level of exposure,” noting that side-stream smoke carries higher concentrations of unfiltered toxic and carcinogenic compounds.

In India, health experts and the citizen-led initiative Tobacco Free India are advocating for the implementation of 100% smoke-free public spaces. Citing the same Global Burden of Disease data, experts noted that an estimated 325,600 deaths in India in 2023 were attributable to second-hand smoke exposure, including approximately 150,000 women and 175,000 men. While the proportion of the Indian population exposed has decreased from 43% in 1990 to 31% recently, the absolute death toll remains high due to significant population growth. The push seeks to move beyond designated smoking areas toward entirely smoke-free environments to protect non-smokers.
