Japan health insurance coverage reforms

The Japanese government is planning to revise health insurance coverage for drugs similar to Over-the-Counter (OTC) medications, with changes scheduled to begin in March 2027. The policy would require patients to pay an additional fee equal to one-quarter of the cost for approximately 1,100 items covering common ailments like inflammation and stomach pain.

Proponents argue the reform is necessary to stop social insurance funds from subsidizing drugs that are easily accessible at drugstores. However, the Federation of National Health Insurance Physicians has criticized the plan, stating it imposes an “unreasonable administrative burden” on medical facilities and could increase patient wait times. The Japan Medical Association has also warned that the increased costs might discourage patients from seeking necessary medical consultations.

Health Minister Ueno indicated that the government is not currently considering financial support for medical institutions to manage the administrative updates required by the new system. Instead, the Ministry plans to provide standards for electronic medical record vendors and simplified checklists for clinics.
