The Portuguese government has issued a decree establishing a 15% state subsidy for obesity medications, set to take effect in February 2027. This policy marks a shift in how these treatments are accessed and funded. Under the new regulations, prescribing authority is restricted to endocrinologists, internists, or pediatricians serving within multidisciplinary obesity teams (EMO) at Local Health Units (ULS). This change effectively excludes primary care physicians from prescribing these medications. To qualify for the subsidy, patients must also be part of an integrated care pathway (PCIPO). The subsidy is limited to a 12-month period from the first dispensing to facilitate therapeutic evaluation. The program mandates a multidisciplinary approach, incorporating nutritional intervention, physical activity planning, and psychological support. Medical professionals and patient advocacy groups have criticized the measure. Nuno Jacinto, president of the Portuguese Association of General and Family Medicine (APMGF), warned the policy could restrict access and strain hospital services. Additionally, Carlos Oliveira, president of the Association of Obese and Former Obese Patients (APEXO/Adexo), stated the 15% subsidy is insufficient compared to other European nations that provide subsidies ranging from 50% to 100%. Critics also noted that the requirement for specialized multidisciplinary teams may create regional barriers to treatment.
