Prostate cancer treatment and diagnostic challenges

Overview

Recent medical research in the United States has highlighted discrepancies between current prostate cancer diagnostic features and treatment efficacy or guidelines.

A UCLA Health study involving 4,781 patients found that traditional pathological features used to classify aggressive disease do not accurately predict whether patients will benefit from adding hormone therapy to postoperative radiation. Researchers noted that while certain high-risk findings can identify patients at risk for poor outcomes, they fail to determine the effectiveness of hormone therapy, suggesting a need for molecular biomarkers to personalize treatment.

Separately, a national analysis published in JAMA involving 15,672 men found that approximately 51 percent of those receiving focal therapy fall outside the risk groups supported by clinical guidelines. The study indicates a mismatch where patients with low, high, or very high-risk disease are receiving targeted treatments that current guidelines from the American Urological Association and the American Society for Radiation Oncology suggest are either investigational or should not be recommended outside of clinical trials.
