Spain’s Ministry of Health is working to expand access to medical abortions by allowing the procedure to be performed in local primary care centers rather than exclusively in hospitals or private clinics. Health Minister Mónica García stated the plan aims to improve continuity of care and reduce travel requirements for women. To facilitate this, the government intends to modify the royal decree regulating health center practices and update the technical data sheet for mifepristone. The Ministry has launched a public consultation process for a royal decree that would include the prescription, monitoring, and management of pharmacological abortion as official functions of Family Planning units. This reform seeks to update regulations that currently focus on surgical procedures to better accommodate pharmacological methods. The proposal also includes organizational mechanisms to ensure that the individual conscientious objection of healthcare professionals does not delay access. In the Basque Country, the health service Osakidetza is launching a pilot program in October for pharmacological abortions of fewer than ten weeks of gestation, with the goal of implementing the service across its entire public network by 2027. In October 2026, the Ministry launched a public consultation for a royal decree to include the indication, prescription, and management of potential complications of pharmacological abortion as official duties for specialists in Obstetrics and Gynecology or Family and Community Medicine. Additionally, the Ministry called an extraordinary meeting of the Interterritorial Council of the National Health System to inform autonomous communities about a draft decree regarding the voluntary interruption of pregnancy. This draft aims to adapt frameworks to Organic Law 1/2023, regulating the information provided for prior consent and distinguishing between mandatory medical information and information provided only at a patient's request.
