Spanish pharmacists advocate for medication safety

Overview

The Spanish Society of Primary Care Pharmacists (Sefap) has initiated efforts to improve patient safety by addressing improper medication management. The organization launched the ‘La necesidad invisible’ campaign to promote medication reviews in outpatient settings and advocate for the identification of “prescription cascades.” These cascades occur when secondary symptoms are misinterpreted as new conditions, leading to the unnecessary addition of medications and increased polypharmacy risks.

Building on these safety initiatives, Sefap president Mercedes Pereira has emphasized the importance of pharmacists reviewing “legacy prescriptions.” These are treatments originally intended for limited durations that persist as chronic medications without adequate re-evaluation. Pereira noted that pharmacists work to identify and propose adjustments—such as the reduction, withdrawal, or substitution of drugs—through coordination with medical and nursing teams, rather than replacing clinical decision-makers.
