The U.S. administration and the FDA have moved forward with initiatives involving biotechnology and drug pricing. Initially, the FDA and industry representatives released the Prescription Drug User Fee Act (PDUFA) VIII commitment letter, establishing a framework for the agency’s review process from 2028 to 2032 focused on stability and transparency. Concurrently, the administration began preparing agreements with midsize biotechnology companies to lower outpatient medicine prices for Medicaid programs by aligning U.S. costs with international prices. This initiative expanded into the Most Favored Nation (MFN) drug pricing plan. By late August, Secretary of Health and Human Services Robert F. Kennedy Jr. announced that 26 companies had entered into voluntary agreements under this plan, covering nine of the ten most commonly used drugs in the United States. On August 31, 2026, President Donald Trump and Secretary Kennedy announced an expansion of the MFN initiative, with nine additional pharmaceutical companies—Alcon, Astellas Pharma, BeOne Medicines, BridgeBio, CSL, Kyowa Kirin, Sun Pharma, Teva Pharmaceuticals, and UCB—entering voluntary agreements. The administration stated these 26 total agreements now cover approximately 90% of the domestic branded pharmaceutical market, a move projected to save Americans more than $600 billion. In exchange for these pricing commitments, participating manufacturers may receive multiyear tariff relief and have pledged a collective investment of at least $19.6 billion in U.S. manufacturing. Furthermore, firms including Astellas, Sun Pharma, Teva, and UCB will contribute active pharmaceutical ingredients to the federal government’s strategic medical reserve to reduce reliance on foreign supplies. While large pharmaceutical firms have negotiated MFN deals to maintain profitability, the expansion of these pricing controls and potential tariffs has raised concerns regarding the innovation and profit margins of mid-sized biotechnology companies.
