A Department of Health and Human Services (HHS) report, titled ‘Wolves in White Coats: How Doctors and Hospitals Pushed and Profited from the Fraud of Gender Medicine’, alleges that over $60 million in insurance claims were fraudulently billed for pediatric gender-affirming care between 2015 and 2025. The report claims that more than 225 hospitals and health systems may have used misleading diagnostic codes, such as ‘precocious puberty’ or ‘endocrine disorder, unspecified’, to secure coverage from Medicaid and private insurers for treatments including puberty blockers, cross-sex hormones, and surgeries for adolescents. Updated findings indicate that hospitals and clinics have billed nearly $120 million for gender-related medical treatments to minors since 2019. Specific allegations include approximately $50 million in claims for puberty blockers billed under an unspecified endocrine disorder code (E34.9) and nearly $11 million in claims for patients aged 13 to 17 billed using a code for precocious puberty. Mount Sinai Medical Center in New York and Boston Children’s Hospital were identified as among the highest billers. Following these findings, Vice President JD Vance and HHS Secretary Robert F. Kennedy, Jr. referred the 225 identified hospitals and healthcare systems to the Department of Justice. Justice and the HHS Office of Inspector General for a federal criminal investigation. In a communication to Attorney General Todd Blanche, Vance stated the administration intends to investigate whether providers violated federal laws or regulations through intentional miscoding to perpetrate fraud. Additionally, the Trump administration has finalized a rule barring two major federal health programs from funding puberty blockers, hormone therapy, and surgeries for gender dysphoria.
