Italy healthcare: Agenas reports mixed results for waiting lists

The latest Agenas bulletin for the first nine months of 2026 shows a national trend of improvement in healthcare waiting lists, though significant regional disparities persist. Nationally, bookings increased from 19.7 million to 21.8 million. Compliance with guarantee times rose to 78.6% for first specialist visits and 85% for diagnostic exams.

Regional performance varies widely. Basilicata leads the country with a 99% compliance rate for first visits and 95.1% for diagnostics. Molise and Abruzzo also report results above the national average, with Abruzzo showing a 9-point increase in visit compliance. Conversely, regions like Umbria and Calabria face significant challenges, with Umbria ranking near the bottom for diagnostic timeliness.

While some progress is noted, advocacy groups like Codici warn that over 4 million appointments were still provided outside of guaranteed timeframes. This delay has contributed to an increase in citizens renouncing care, with 9.9% of the population reporting such instances in 2024. In response to these systemic issues, Sardinia has approved a new regional plan for 2026-2028 aimed at implementing a unified booking system (CUP) to better manage public and private healthcare availability.
