Germany faces rising costs and challenges in long-term care system
Germany is facing significant challenges in its long-term care system, characterized by rising costs and administrative hurdles. Recent data from the Medizinischer Dienst Bayern indicates that more than one in four initial applications for a care level (Pflegegrad) are rejected, as a need for help does not always meet the legal definition of care dependency.
Financially, the burden on both individuals and municipalities is increasing. In Munich, the median out-of-pocket expense for inpatient care rose by 6.7 percent to 3,833 euros. In the Rhein-Kreis Neuss region, expenditures for social assistance for care have increased by 22 percent since 2023 due to rising facility costs. In North Rhine-Westphalia, the number of people receiving social assistance for care has risen for three consecutive years.
Legal and regulatory changes are also impacting recipients. While a new regulation for 2026 will reduce the frequency of mandatory consultation visits for those with care levels 2 through 5, legal rulings have clarified that private insurers may offset outstanding contribution debts against care allowance (Pflegegeld). Additionally, health data from the AOK indicates regional disparities in chronic conditions, such as heart failure, which affects approximately 4.8 percent of the population in Schleswig-Holstein.
