Telstra investigation reveals network timing failures caused massive outage

An external investigation by Technology Audit Partners (TAP) has identified critical failures within Telstra that led to a massive nationwide mobile outage in July. The investigation found that the disruption was triggered by an incorrect date being propagated through the network following a planned system update. Specifically, a GPS card in a timing chassis reset and incorrectly reported the year as 2006 due to a missing firmware update.

The report concluded that Telstra failed to treat network timing as a critical capability, citing insufficient technical expertise, poorly defined ownership of the function, and weak process discipline. Notably, the two engineers with deep expertise in the network time protocol (NTP) system were on mandatory standdown when the impact of the change became known.

The outage had severe consequences, including the disruption of digital payments, halting rail services, and preventing hundreds of emergency Triple Zero calls from connecting. Telstra CEO Vicki Brady admitted the company failed to prioritize network timing as a critical capability. The incident has prompted a Senate inquiry and a regulatory investigation that could result in fines of up to $30 million.
