Incheon districts expand integrated community care networks

Local districts in Incheon, South Korea, are expanding integrated community care systems to support residents following hospital discharge.

In Yeongjong-gu, the district office has signed agreements with five new service providers, including the Incheon Branch of the Physical Therapy Association and Samsung Nursing Hospital. This expansion aims to provide specialized services such as home-visit exercise, mobile bathing, and hair/beauty services to address diverse care needs. The initiative seeks to bridge the gap between medical treatment and daily life, allowing residents to return to their communities more stably.

Similarly, Geomdan-gu has expanded its medical cooperation network by adding three new hospitals—Geomdan Top Hospital, Onnuri Hospital, and Gimpo Woori Hospital—bringing its total to nine partner institutions. This network is designed to identify patients requiring continuous medical and nursing support early in the discharge process. The goal is to create a seamless medical-welfare model that prevents care gaps for the elderly and disabled, ensuring they can live healthily in their familiar environments.
