Doctors in Italy resign at record rates, surpassing retirements in 2025

A report by the Centro Studi Anaao Assomed reveals a significant shift in the Italian National Health Service (SSN) during 2025. For the first time in the 2015-2025 decade, voluntary resignations of hospital doctors (3,767) have surpassed retirements (2,876). This represents a 150% increase in voluntary resignations compared to the 1,503 recorded in 2024, bringing the incidence to 3.5% of all public health doctors.

The exodus is not limited to those nearing retirement; the average age of resigning doctors is approximately 48 years, indicating a loss of clinically mature professionals to the private sector or positions abroad. In 12 out of 20 Italian regions, resignations have exceeded retirements, with the most critical situations found in Lazio, Trentino-Alto Adige, Friuli-Venezia Giulia, and Sardegna, where resignations were 2.4 to 3.3 times higher than retirements.

Anaao Assomed cites heavy workloads, low salaries compared to European standards, excessive bureaucracy, burnout, and workplace aggression as primary drivers of this trend. The report notes that for every 100 doctors hired on a permanent basis in 2025, 43 resigned and 77 left the public system in total.
