New markers improve arrhythmia risk detection in dilated cardiomyopathy

An international study led by researchers from the Centro Nacional de Investigaciones Cardiovasculares (CNIC) and the Hospital Universitario Puerta de Hierro Majadahonda has identified new markers to improve risk stratification for severe ventricular arrhythmias in patients with non-ischemic dilated cardiomyopathy.

Published in the journal Circulation, the research involved a cohort of 925 patients from 22 hospitals across Spain and the Netherlands. The study demonstrates that combining genetic information with advanced cardiac magnetic resonance techniques allows for more precise prediction of arrhythmia risks than current clinical criteria.

Currently, the decision to implant preventive defibrillators relies primarily on left ventricular ejection fraction, a measure of heart pumping capacity. However, this method has limitations and fails to adequately identify patients at risk; the study found that nearly half of arrhythmic events occurred in patients whose cardiac function was above the high-risk threshold. The new approach focuses on cardiac fibrosis characteristics detected through gadolinium-enhanced magnetic resonance imaging.
