Poland Implements New E‑Health Home Care System for Chronic Disease Monitoring

A government bill amending several statutes to develop e‑health services received presidential approval in June. The law creates a Home Medical Care (DOM) platform that will let patients with chronic conditions such as hypertension and diabetes record measurements from smart devices or manually via the national health portal. Doctors can access these data, but the system does not impose a mandatory review frequency, leaving the schedule to the treating physician. The rollout is required within five months of the law’s entry into force and will initially cover cardiology, diabetology and preventive care, integrating with services like Google Health and Apple Health.

Separately, the Pisz County administration, together with a Lithuanian primary‑care centre and the local hospital, launched an EU‑funded project titled “Dignity and Care – Help in Dementia and Diabetes”. The Interreg Lithuania‑Poland programme financed the creation of 16 educational articles—eight on dementia and eight on diabetes—aimed at raising public awareness and supporting patients and caregivers. The materials are now available online and in local press.
