Poland healthcare: funding crisis, protests & reforms

Overview

Poland’s healthcare system continues to undergo significant digital, financial, and structural transformations. To manage a 14 billion PLN budget deficit, the National Health Fund (NFZ) has been shifting payments for services performed beyond established limits to future fiscal years, prioritizing oncology and childbirth. New diagnostic funding rules implemented on April 1, 2026, have led to a significant increase in waiting times for essential examinations. According to data from Alivia Onkofundacja, patients waiting for a non-anesthetized colonoscopy in regular mode now face an average wait of 336 days, an increase of 99 days. Urgent cases face an average wait of 262 days, up by 94 days. These delays follow NFZ changes where, for services exceeding contract values, the NFZ pays only 50 percent of the rate for CT scans and MRI, and 60 percent for gastroscopy and colonoscopy. This reduction in profitability has reportedly prompted some facilities to limit patient intake or suspend registrations. Gastroscopy waiting times have risen to 200 days in regular mode. Financial strain is intensifying at the local level. The Zywiec County Council has appealed for urgent reforms to stabilize district hospital funding, noting that Zywiec Hospital has faced unpaid services for 2024–2025 totaling approximately 55 million PLN, despite service surpluses in surgery, pediatrics, and diagnostics. Concurrently, the Polish Nurses and Midwives Union (OZZPiP) is protesting cost-cutting measures by hospital directors. The union claims hospitals are attempting to offset recent wage increases by reducing working hours and cutting staff positions, stating that medical staff should not serve as a “financial buffer” for systemic funding issues. Regarding sanatorium treatments, patient co-payments for accommodation and meals are decreasing for the autumn-winter season starting October 1, 2026. For a 21-day stay, patients could see savings exceeding 200 PLN depending on room standards.
