Dementia risk varies significantly between leisure exercise and strenuous work

A large-scale meta-analysis published in The Lancet Public Health reveals that the context of physical activity significantly influences dementia risk. Researchers analyzed data from 74 studies involving over 4.2 million participants across 30 countries, finding a distinction between voluntary leisure-time exercise and occupational physical activity.

Leisure-time activities, such as running, cycling, and swimming, were associated with a 24 percent reduction in dementia risk. In contrast, physically demanding jobs involving heavy lifting or long periods of standing were linked to a 20 percent increase in risk. This phenomenon, known as the ‘physical activity paradox,’ suggests that the neurobiological benefits of movement are influenced by factors like stress, social interaction, and environmental exposures.

While leisure exercise provides protection, strenuous work may involve repetitive tasks, limited recovery, and exposure to noise or pollution, which could contribute to cognitive decline. Other research suggests that combining mental tasks with physical movement may be more effective for brain resilience than cognitive games alone.
