Advancements in porcine xenotransplantation
Overview
Medical researchers have achieved significant milestones in xenotransplantation, utilizing genetically modified porcine organs to address human organ shortages.
In one instance, a Combined Liver-Kidney Transplantation (CLKT) was successfully performed on a 53-year-old brain-dead patient. This procedure involved the simultaneous transplantation of a genetically modified pig liver and kidneys. To prevent immune rejection and coagulation issues, scientists edited six porcine genes to silence human-triggering antigens and express human regulatory proteins. The organs maintained physiological functions, such as bile production and toxin filtration, for a five-day monitoring period.
Recent advancements in kidney transplantation have established new benchmarks for longevity. Patient Tim Andrews, a 66-year-old man with end-stage kidney disease caused by type 2 diabetes, received a kidney from a Yucatan miniature pig in January 2025 at Mass General Brigham. The organ underwent 69 genetic edits to mitigate viral risk and immune rejection. According to findings published in The Lancet, the kidney functioned for 271 days, allowing Andrews to avoid dialysis for approximately nine months. While no pig-borne pathogens were detected, the organ eventually failed due to vascular damage and inflammation, leading to its removal in October 2025. Following 82 days of dialysis, Andrews successfully received a kidney from a deceased human donor in January 2026. This case marks the first successful transition from a functioning pig-to-human kidney transplant to a subsequent human organ transplant.
Building on this, biotech company eGenesis reported that another patient has lived with a transplanted pig kidney for over nine months (approximately 285 days) without requiring dialysis. This second case sets a new record for the duration of pig kidney function in a human recipient. Researchers suggest these modified organs may serve as a “bridge” for the approximately 100,000 people in the United States currently awaiting human kidney transplants.
