In Ontario, the Ontario Health Coalition has criticized the provincial government for allocating hundreds of millions of dollars to private clinics for MRI, CT, endoscopy, and orthopedic services, arguing these funds should instead bolster public hospital capacity. Meanwhile, Alberta has implemented Bill 11, which allows physicians to provide publicly funded care alongside services for private payment to help patients avoid wait times.

In Quebec, the discussion has focused on the role of the private sector in surgical care. Critics have challenged studies suggesting private surgeries are more cost-effective, arguing such studies fail to account for the complexity of cases handled by public hospitals. In the Outaouais region, concerns have been raised that reliance on private clinics and the transfer of patients could exacerbate local health crises and contribute to medical staff shortages. Conversely, proponents of a mixed model suggest that regulated private sector involvement could address resource shortages and reduce the need for patients to pay out-of-pocket for care in other cities.
