Researchers at East China Normal University, led by Professor Ye Haifeng, have developed a genetically engineered probiotic named GIFT to manage diabetes. Published in the journal Nature, the study describes a ‘living medicine’ that utilizes a synthetic genetic circuit to act as a glucose sensor within the intestine.

Unlike traditional GLP-1 medications that provide fixed doses, the GIFT probiotic employs a ‘demand-based’ or ‘detect and respond’ mechanism. The modified Escherichia coli bacteria release a GLP-1 hormone analogue only when blood glucose levels exceed a specific threshold, automatically ceasing production once levels normalize. This approach aims to reduce side effects associated with continuous hormonal exposure.

In experimental trials involving mice and non-human primates, the probiotic effectively controlled blood sugar and improved lipid levels. Specifically, in trials with diabetic monkeys, a single oral dose administered every three days maintained stable blood sugar for five consecutive weeks. To mitigate risks, the bacteria are designed to colonize the intestine only temporarily so they are naturally eliminated from the body. The research team has applied for patents in China, the US, and Europe.

Recent scientific discussions have highlighted a divide between the popularity of probiotics and the lack of conclusive evidence in large-scale clinical trials for general health benefits. Regarding the specific development of glucose-regulating probiotics, experts note that while this reactive method—designed to intervene only during hyperglycemic episodes—is promising for type 2 diabetes management, the technology still requires extensive preclinical and clinical testing to ensure long-term efficacy and safety.
